SAR Incident Cause

Incident Causes define what caused the incident - or in the case of uncertainty what was thought to be the cause (the best guess)

Indicate the cause the following choices are standard items and their descriptions. Avoid the use of ´other´ except in cases where none of the other choices could possibly be selected.

Bad Condition (Negligence)
Dry rot, rust, un-repaired damage, vessel is in poor condition, etc.

Dangerous Piloting
Incident caused by the operator to comply with navigation rules, by excessive speeding, undue care and attention, etc.

EPIRB Accidentally Activated
Self-explanatory

Failure to Report.
When a vessel/boat is overdue with a CGRS/VTS/ECARE/NORDEG or WESTREG.

Fatigue
Self-explanatory

Hoax
False message/distress signal. (NOTE: Only to be used when the conveying of information is done with the intent to deceive.) Any other case wherein the subject is confirmed not to be in distress and the intent was not to deceive should be classified as "Mistaken Belief"

Hull/Rigging Failure
Any hull or rigging failure not caused by negligence

Ice
Vessel´s water intake is blocked by ice, vessel is crushed or dragged by ice.

Illness
Self-explanatory (does not include suicides/suicide attempts)

Injury
Injury resulting from the incident, hypothermia, exposure to cold, etc.

Lack of Knowledge
Operator does not know the buoyage system, cannot operate his/her navigational instruments (radar, sounder, GPS, etc.), not familiar with area, etc.

Mechanical Failure
Any mechanical breakdown of main propulsion system not resulting from negligence; Breakdown of the shaft and the propeller, including transmission; Failure of the rudder system
(cables, pump, hoses, etc); Any other breakdown, other than previously mentioned, such as general breakdowns caused by obstruction of water intakes, electrical or electronic breakdowns, and structural damage.

Mistaken Belief
When the incident was triggered by a mistaken belief or an over-reaction by the people reporting it, the vessel/persons having never been in any danger/difficulty.

Navigational Error
Incident caused by miscalculation of the position, by having no marine charts on board, by a false maneuver/lack of lookout

Oil Rig Evacuation Alert
When an oilrig is evacuated as a preventive measure.

Out of Fuel (Negligence)
Out of fuel (main propulsion system)

Overload/Stability
Vessel is overloaded or questionable stability

Propeller Fouled
Caused by nets, moorings, or fishing lines caught in the propeller

Suicide or Attempt
Self-explanatory

Weather
Incident caused by foul weather conditions or failure by the operator to obtain weather information

Unknown
When the cause of the incident cannot be determined

Other
Any other cause not previously identified. (Specify in comments section)
